Management of Refractory Gastroesophageal Reflux Disease

Abstract
Refractory gastroesophageal reflux diseases (GERD) is generally defined as a condition which is not properly controlled with twice daily doses of proton pump inhibitors (PPI) for 4¡­8 week. The pathogenesis underlying refractory GERD can be either from failure of adequate acid suppression and from the other cause resulting in similar symptom with GERD. Consideration about the cause of failure and other possible diagnosis leads to in-depth examinations including endoscopy, esophageal manometry, impedance test, and so on. However, the most important things to do with refractory GERD are a thorough history taking and the identification of drug compliance.
